When filling the incorporation documents, it should be group and arrange the grouped documents in the following order :

1st Group             :        Name Verification Slip - 1 copy

2nd Group            :        D.O.L.E Form - 1 copy

3rd Group            :         Bank Certificate of Deposit
                                        -    4 copies
                                        -     attach one documentary stamp to original copy

4th Group             :        Articles of Incorporation with Coversheet
                                        -    4 sets ( each set is arranged from coversheet, pages 1 to 6)
                                        -    attach endorsement letter on 1st set, if applicable

5th Group            :        Treasurer's Affidavit
                                        -    4 copies
                                        -    attach one documentary stamp to original copy

6th Group            :        Confirmation of Corrections
                                        -    4 copies
                                        -    attach one documentary stamp to original copy

7th Group            :        By-Laws
                                        -     4 sets ( each set is arranged from pages 1 to 9 )

Yes. Just have it copied. All docs will be in 4 original copies
